3. Fill Out Your Forms
Once you set up your student portal, you’ll be able to fill out some forms that we need from you before you arrive on campus. Please note that all forms must be submitted by May 1.
Personal Profile Form
We’re committed to helping you achieve your academic goals and stay on track to graduate on time. That’s why we ask you to fill out a Personal Profile form. This form helps us put together an academic profile of you.
We are dedicated to providing a healthy environment for you at PLNU. In order to ensure this, you'll need to provide some medical information and have your doctor assist you with completing certain forms. The required forms can be filled out by you and your doctor, and include information about your health history, immunization records, and more. We'll also need copies of your immunization records and health insurance card, as well as results of a recently administered physical exam by your doctor. All of this information and more can be found in your student portal.

Read our Terms and Conditions of Occupancy.
You will get your official housing information (including where you’ll live, and who your roommate will be) in early December (for spring students) or July (for fall students) on your Student Portal.
